Couple Checkup Privacy Policy

PREPARE/ENRICH, LLC. is committed to guarding the privacy of any customer
information gathered and transmitted over the internet. For over 25 years,
PREPARE/ENRICH has demonstrated a solid track record of being sensitive to
and respectful of confidential information. This privacy policy is intended
to inform you of what information is collected, how it is used, and kept
secure.
Information Gathered and How it is Used
A. Payment Information: Before creating your account to take the inventory
online, a couple without a pre-assigned voucher must provide payment.
Credit Card information can be entered on the secure encrypted payment
section of the Site. Payment information is used only for payment
purposes.
B. Once a couple has purchased a voucher to take the Couple Checkup,
they create their own username and password, and are provided with a computer
generated account number. This allows both members of a couple to Login to a
secure website and respond to the Couple Checkup items.
1. Inventory Items: Reponses are recorded numerically. Couple responses are stored
in a secure password protected database and used to calculate inventory results.
2. Background Items: Many of the responses to Background items are asked in
ranges (i.e. Age: 20-25, 26-30 . . .) which further protects individuals from
providing identifiable information. The Background and demographic items are not
required for the scoring of the Couple Checkup.
3. Email Addresses: Email addresses of couples are stored securely
by PREPARE/ENRICH and are not shared or sold to third parties. Email is used for login purposes only, unless
individuals choose to receive email communications and news letters from PREPARE/ENRICH, LLC.
Research
PREPARE/ENRICH stores archival data on couples who have completed the Couple Checkup.
This data is stored on a secure password protected database. No names or
identifiable information are connected to these records. PREPARE/ENRICH, LLC.
psychologists and behavioral specialists use these records for anonymous research
purposes only and are trained in protecting confidential information as well as
handling secure data.
Groups
If you are taking the Couple Checkup as part of a group or event, you may be prompted to enter a
group number. Your participation is not required. Your name, email address, or other
identifiable information will not be shared or accessible to the group leader. Group summaries
can only be generated after 5 or more couples in a group have completed their Couple Checkups.
Group summary reports only display averages for the entire group. Information for individual
couples is not viewable or reported on.
Information
When you visit this site certain types of information about your visit are automatically
captured. This may include the date and time you access this site, or the pages that you
access while at this site. None of this information is personally-identifiable so each user
will remain anonymous. This information helps PREPARE/ENRICH, LLC. to understand which
portions of our site users are visiting most frequently.
Cookies
Cookies are not required in order to use our site. If you do have cookies turned on in your browser,
our site will only use them to store a randomly generated session id. If you do not have cookies
turned on in your browser, our site will automatically include a randomly generated session id within
the URL for each page that you access. In either case, the session id allows your browser to maintain
a continuous user session with our site and is used for no other purpose.
Age Requirements
The Couple Checkup administered and scored online is for adults 18 years of
age and older. PREPARE/ENRICH does not knowingly collect any information from minors,
and will purge this information from our database if discovered.
Security
PREPARE/ENRICH, LLC. has adopted security measures to protect the loss, misuse and
alteration of all information stored in our database. These measures include the use
of Secure Server technology during credit card transactions and administrative access
to site data, as well as other proprietary security measures. While we are committed
to exercising reasonable care in providing secure transmission of information between
your computer and our servers, we cannot ensure or warrant the security of any information
transmitted to us over the Internet and we accept no liability for any unintentional disclosure.
